What Information will i need?

Place of Birth.

Date of Birth.

|
|
Baby's full name.
|
|
|
Baby's sex.
|
|
|
Baby's weight.
|
|
|
Baby's parents full name and place of birth.
|
|
|
Baby's mothers maiden surname or previous names.
|
|
|
Baby's parents address.
|
|
|
Baby's parents occupation.
|
|
If you are having twins, triplets etc, you will need
to provide information of time of each baby is also needed.

What Happens Next?
|
|
|
The Registrar will record the details in the birth register and into
a birth registration computer file.
|
|
|
You will need to check the information to make sure it is correct and
then sign the register.
|
|
|
You will be given a certificate and a prelimminary medical card to enable
baby to be registered with the Doctor.
|
|
|
There is no charge for this unless you want additional memento's.
